TLV Physio provides physiotherapy treatments delivered in-person at our clinic (Nachmani 28, Tel Aviv) or at the patient's home by prior arrangement.
TLV Physio מספקת טיפולי פיזיותרפיה הניתנים באופן פרונטלי במרפאה (נחמני 28, תל אביב) או בבית המטופל בתיאום מראש.
2. Age | גיל
Our services are available to patients of all ages. Patients under the age of 18 must be accompanied by a parent or legal guardian, who must provide consent for treatment on the minor's behalf.
השירותים שלנו פתוחים למטופלים מכל הגילאים. מטופלים מתחת לגיל 18 חייבים להגיע בליווי הורה או אפוטרופוס חוקי, אשר יידרש לחתום על הסכמה לטיפול בשם הקטין.
3. Cancellation Policy | מדיניות ביטול
Cancellations must be made at least 24 hours before the scheduled appointment. Cancellations made less than 24 hours in advance will be charged the full session fee. TLV Physio reserves the right to cancel or reschedule appointments where necessary, with as much advance notice as possible.
ביטול תור חייב להתבצע לפחות 24 שעות לפני מועד הטיפול המתוכנן. ביטול שנעשה פחות מ-24 שעות לפני התור יחויב במחיר מלא של הטיפול. TLV Physio שומרת לעצמה את הזכות לבטל או לשנות תורים במידת הצורך, תוך מתן הודעה מוקדמת ככל האפשר.
4. Payment & Token Storage | תשלום ושמירת פרטי כרטיס
Payments are processed securely through Grow | Meshulam, a licensed Israeli payment provider. Credit card details are encrypted and stored securely by Grow | Meshulam. TLV Physio does not store your full credit card details. Where you have consented to saving your card details for future sessions, your encrypted card token may be used to charge future appointments automatically. You may request removal of your stored card details at any time by contacting us on WhatsApp at 054-984-7300.
התשלומים מעובדים באופן מאובטח דרך Grow | Meshulam, ספק תשלומים ישראלי מורשה. פרטי כרטיס האשראי מוצפנים ומאוחסנים באופן מאובטח על ידי Grow | Meshulam. TLV Physio אינה שומרת את פרטי כרטיס האשראי המלאים שלך. במקרה שנתת הסכמתך לשמירת פרטי הכרטיס לטיפולים עתידיים, ייתכן שהטוקן המוצפן של כרטיסך ישמש לחיוב תורים עתידיים באופן אוטומטי. ניתן לבקש הסרת פרטי הכרטיס השמורים בכל עת על ידי פנייה אלינו בוואטסאפ: 054-984-7300.
5. Privacy | פרטיות
We collect and use your personal information solely for the purpose of providing physiotherapy services, managing appointments, and processing payments. We do not sell or share your data with third parties for marketing purposes. For full details see our Privacy Policy on this website. You have the right to request deletion of your personal data at any time by contacting telaviv.physio@gmail.com.
אנו אוספים ומשתמשים במידע האישי שלך אך ורק לצורך מתן שירותי פיזיותרפיה, ניהול תורים ועיבוד תשלומים. איננו מוכרים או משתפים את המידע שלך עם צדדים שלישיים למטרות שיווק. לפרטים המלאים ראה את מדיניות הפרטיות שלנו באתר זה. יש לך זכות לבקש מחיקת המידע האישי שלך בכל עת על ידי פנייה אל telaviv.physio@gmail.com.
6. Liability | אחריות
TLV Physio's physiotherapists are fully qualified and registered professionals. Treatment is provided based on clinical assessment and professional judgment. TLV Physio shall not be liable for any indirect or consequential damages arising from treatment. Patients are responsible for disclosing all relevant medical history and conditions prior to treatment.
הפיזיותרפיסטים של TLV Physio הם אנשי מקצוע מוסמכים ורשומים. הטיפול ניתן על בסיס הערכה קלינית ושיקול דעת מקצועי. TLV Physio לא תישא באחריות לכל נזק עקיף או תוצאתי הנובע מהטיפול. המטופלים אחראים לגלות את כל ההיסטוריה הרפואית והמצבים הרלוונטיים לפני הטיפול.
7. Business Cancellation Rights | זכות ביטול עסקה
TLV Physio reserves the right to cancel any transaction and issue a full refund where the service cannot be provided, including due to practitioner illness, force majeure, or circumstances beyond our control.
TLV Physio שומרת לעצמה את הזכות לבטל כל עסקה ולהחזיר את מלוא התשלום במקרה שלא ניתן לספק את השירות, לרבות עקב מחלת המטפל, כוח עליון או נסיבות שאינן בשליטתנו.
8. Consumer Protection | הגנת הצרכן
These terms are governed by the laws of the State of Israel, including the Consumer Protection Law, 1981 (חוק הגנת הצרכן, תשמ"א-1981). Any dispute arising from these terms shall be subject to the exclusive jurisdiction of the courts of Tel Aviv.
תנאים אלו כפופים לחוקי מדינת ישראל, לרבות חוק הגנת הצרכן, תשמ"א-1981. כל מחלוקת הנובעת מתנאים אלו תהיה כפופה לסמכות השיפוט הבלעדית של בתי המשפט בתל אביב.
